To prepare for your evaluation, should should organize the following evidence:
- The full name of your last employer
- A Copy of your last W2 or 1099
- A copy of any recent employee evaluation
- The last date that you physically worked and the date of any employment termination
- Your written position description
- Any written termination documentation
- Copies of all documetation received from the Social Security Administration or a disability insurance provider
- Any recent medical records for FMLA or disability issues, including the names of your physicians
- Any unemployment paperwork received from the VEC or the Workers' Compensation Commission
- Copies of any relevant employment policies or union contracts
- The names and contact information of any relevant witnesses
